Forklift License Requirements

In 1999, OSHA revised their powered industrial truck guidelines to require all operators attend a forklift safety training class that includes formal instruction, hands-on training and an evaluation of the operator’s performance on the worksite. Conducted by a trainer with knowledge and experience, the forklift training must be received upon hire and renewed every 3 years.

OSHA outlines the certification required for each operator, which must be kept on file by the employer. Per 1910.178(l) “the employer shall certify that each operator has been trained and evaluated”. The certification must include:

  • Name of the operator
  • Date of the training
  • Date of the evaluation
  • Identify of the person(s) performing the training or evaluation

In addition to keeping the operator’s certification on file, some employers find it beneficial to issue wallet cards that include these details as well as the type of forklifts the operator is authorized to drive. Wallet cards can be easily accessed and shown to a supervisor or OSHA rep during worksite audits.
